An exciting new opportunity has been created for the right person to join the City Intermediate Care Team, a highly-motivated, busy, multi-disciplinary team enabling older people to maintain their independence at home. The post-holder will work across Stoke on Trent.

Job Title: Social Rehabilitation Co-ordinator (City Intermediate Care Team)

Salary Scale: £19,436 per annum

Hours: 37 hours per week

Location: Stoke on Trent

Responsible To : Intermediate Care Co-ordinator

Working Relationship
Close liaison and working with the Intermediate Care Co-ordinator, the City Intermediate Care Team Manager and all staff within the Intermediate Care Team. Peer support and liaison with Age Concern colleagues working in other Social Rehabilitation posts

Job Overview

The post-holder will work as a member of the Intermediate Care Team, providing a service to clients across Stoke on Trent.

The purpose of the post is to work in partnership with health and social care colleagues in providing a Social Rehabilitation service to Intermediate Care clients living in their own homes. This service will promote confidence and independence for clients, as well as providing a range of practical and welfare benefits support and advice to clients within an overall time-limited package of care.

Main Responsibilities

  1. To provide a Social Rehabilitation Service to Intermediate Care clients, working closely with the Intermediate Care Co-ordinator and with health and social care colleagues in the City Intermediate Care Team.
  2. To identify personal aims and goals with clients (involving as appropriate their carers / family) and actively to promote and encourage confidence, independence and skill development, in order to achieve these aims and goals.
  3. To liaise closely and effectively with other members of the City Intermediate Care Team, participating in team meetings and case review meetings as appropriate.
  4. To provide welfare benefits support and advice, assisting clients to secure their entitled benefits.
  5. To develop and maintain good liaison and networking with a range of colleagues from statutory and voluntary service providers, in order to make onward referrals as appropriate.
  6. To develop and maintain appropriate referral systems, documentation for case records and filing systems, ensuring compliance with data protection and confidentiality requirements.
  7. To liaise with the Intermediate Care Co-ordinator in the production of regular reports as required on referrals, activity, progress and outcomes for Age Concern North Staffordshire and for the Intermediate Care Team Manager.
  8. To undertake appropriate training, education and development as identified by Age Concern North Staffordshire and / or the Intermediate Care Team Manager.
  9. To provide cover for Social Rehabilitation colleague(s) within the City Intermediate Care Team during periods of absence as required.
  10. Any other duties appropriate to the role of Social Rehabilitation Co-ordinator as required.

This Job Description will be reviewed annually and may be amended following discussions with the post-holder, to take into account changes and developments.
